First と in その者達が死ぬと彼の許へと還り is the result marker that marks the result/change of an action (people dying) with 者達 being the subject. What returns is the 坊の魂の欠片 and 者達が死ぬ is the actions that caused it, especially since the previous sentence/page in >>274497591 ends with そして which implicates the connection between those.

While その者達が死ぬと is indeed a marker of what causes the return, 赤ん坊の魂の欠片 is not what is returned, it's what being 刻まれ (engraved) to relieve people of wounds, illnesses, etc. And those その者達 who were engraved with 赤ん坊の魂の欠片 return to Yhwach upon death, healing him in turn.

Upon closer inspection, they indeed are. Still, I guess that the main problem there is それら. Which, in my opinion, refers not to soul fragments, but to powers and knowledge.

I don't really want to shit talk someone who's doing volunteer work for the community, and you translated it pretty much perfectly (I'm not JLPT N1, if you are, then I guess I have to concede immediately), but you forgot that Japanese is a language of context, and this whole page is about Yhwach's soul parts that he engraves, and その者達が死ぬと彼の許へと還り just elaborates on the more on the main topic (like what happens to those fragments when those people die), which is why it's so hard to translate Japanese into English without having to change how whole sentences work if you want to keep the overall meaning. Not to mention that other events in the manga don't support the theory that Yhwach literally absorbs everyone's souls.
Again, I'm too tired, to the point of making spelling mistakes, so maybe I'll try to figure out the raws on these two pages later.
